An Intake Specialist in a home health company serves as a critical first point of contact, facilitating the patient intake and referral process with precision and care. The ideal candidate will be responsible for referral management, patient information processing, order management, patient care coordination and ensuring the smooth operation of our home health care services.

 

Responsibilities:

 

Referral Management

• Evaluate incoming referrals for admission appropriateness

• Obtain verification of patient benefits and demographics

• Collect all required clinical support documentation

• Notify corresponding marketers about new referrals

 

Patient Information Processing

• Complete intake information sheets for pending Start of Care (SOC)

• Accurately enter patient information into Electronic Medical Record (EMR)

• Schedule SOC OASIS and related tasks for the SOC Registered Nurse

• Scan and attach clinical support documentation to SOC OASIS tasks

• Verify patient eligibility

 

Order Management

• Follow up with clinicians to ensure timely completion of notes and signing of orders

• Call and fax doctors' offices for order signatures and sending referrals

• Ensure all pending tasks are completed to initiate the billing process

• Advocate for clients with advice and information about treatment options and clinical status

• Process and track physician orders

• Verify order completeness and accuracy

• Ensure timely communication of orders to appropriate clinical staff

• Maintain organized order documentation in patient records

• Follow up on incomplete or missing orders

 

Patient Care Coordination:

• Call and confirm visit timings with patients

• Staff, coordinate, and schedule visits with Physical Therapists, Occupational Therapists, Speech Therapists, and Home Health Aides

• Maintain availability for phone calls and text messages to provide swift responses and address patient issues

• Triage calls between administrative/scheduling staff and patients to confirm visit dates according to patient availability and convenience

• Update and relay patient visit information periodically through Home Health Notify

 

Required Skills and Qualifications:

 

• Proficient in English Speaking skills and adaptability to handling different softwares/EMR

•Degree in Pharmacy, Pharm-D, Nursing or MBBS.

• One or more years of experience as an Intake Specialist, case management, or home health care coordination

• Strong knowledge of psychological aspects of patient care

• Excellent interpersonal, verbal, and written communication skills

• Superb critical-thinking, organizational, and analytical skills

• Ability to work with diverse age groups and populations

• Proficiency in healthcare management software and electronic health records systems
