Description:
1. To screen & diagnose people having diabetes mellitus, pre diabetes & essential hypertension – annexure 1
2. To screen & diagnose GDM (Gestational Diabetes Mellitus) – annexure 1
3. To investigation people having diabetes, hypertension & obesity – annexure 2
4. To register after allocating proper diagnosis:
· Diabetes Mellitus Type 1
· Diabetes Mellitus Type 2
· Gestational diabetes Mellitus
· Pre diabetes ( impaired fasting glucose IFG, impaired glucose tolerance IGT)
· Essential hypertension
· Obesity
5. To provide awareness on
§ Life style modification
§ Role of diet
§ Role of exercise
§ Smoking cessation
§ Foot care
§ Insulin injection technique
6. Follow up protocol – annexure 2
7. Achieving target – annexure 1
8. To prescribe medicine & / or insulin if needed
9. To maintain medical record of all patients
10. To create awareness in general public / screening of new cases
§ Holding free camps (free sugar, BMI, Cholesterol, HbA1c, Micro albuminuria etc.)
§ Family awareness
