Clinical Pharmacist

Description:

The Pharmacist is responsible for facilitating clients in person or online. Ensure safe medication use, monitor patient

history, and stay updated on advancements in medicine. Work with a team, including dietitians & nutrition, to plan

the best medication action to help clients to achieve their health goals.

Key Responsibilities:

 Serves clients by prescribing medications, and giving pharmacological information to multidisciplinary health

care teams in DWF.

 Monitoring clients' drug therapies with the health team.

 Listen carefully to clients to interpret their needs and issues and offer information and advice on

medications.

 Handling client’s queries and providing remote support where needed.

 Controls medications by monitoring drug therapies; and advising interventions.

 Provides pharmacological information to clients by answering questions and requests of health care

professionals and counseling clients on drug therapies.

 Identifying potential interactions between drugs and providing information to the clients.

 Perform regular client reviews to identify and address any medicines-related issues and ensure clients get

the best outcome from the medicines prescribed.

 Make appropriate recommendations to the healthcare team for clients' ongoing treatment and the

management of referrals.

 Comply with all applicable legal rules, regulations, and procedures.

Required Qualification & Experience:

 Bachelor's Degree in Pharmacy/ Doctor of Pharmacy (PharmD).

 Preferably, professional registration with the Pharmacy Council of Pakistan

 Minimum of 2-3 years of experience in a similar role in the health industry.
